I gotta say...as soon as the story was "you take the pills; the game begins," it was like, oh, yeah, he's out of his mind on pills and killing everyone in the hospital, big twist ending, okay.

I think, if we interpret the final ending as the intended one, the whole murder spree aspect of earlier endings is not literal. Its a metaphor for the emotional destruction David’s caused everyone around him (his family, friends, people who loved him) from his addiction, and the guilt of that leads to his eventual suicide.

Seeing the final ending where David overcomes his addiction and wakes back up made me realize how obvious the other endings are with his fate

If you enjoy obscure horror mods, you should check out "Total Chaos" for Doom 2. I have never had something made by such a small dev team scare the shit out of me and mess with my brain the way it did. Pyro did a deep dive into it if any of you want to learn more. Anyway, coming home from a 12 hour shift to new Nexpo always feels good.
