City of Boston: "I'm sorry Cartoon Network, but a few of our citizens baselessly assumed your Light Brites of cartoon characters shaped like Atari graphics were bombs rather than the obvious decorative light-up display advertising pieces they actually were and this lead to a huge chain of bad decisions on the part of many, many people which cost us a lot of money and so this is all clearly your fault for trying to scare us with fake bomb threats or something I guess so cover our costs or go to jail."

The aqua teen one is so fitting. The Mooninites are canonically lame, but think they are evil incarnate super geniuses. At one time they send out a beeping square that looks lame like usual but nukes everything. People thought this was some sinister plot but it was stupid lights, it looked lame but it nuked CN.

the biggest publicity stunt failure i know of would probably be the "Pepsi Number Fever" incident in the phillipines. a mistake in a sweepstakes event to win cash prizes based on a number on pepsi brand bottle caps lead to protests, death threats, riots, and 2 bombing attacks that killed 5 people.
